When you join Star Community Care, we want to hear your story and help you begin a new chapter. That’s why we come to you to get to know you in the comfort of your own home. We offer a complete customized support plan where we identify your outcomes or goals and your challenges. We help customers, like you, understand your plan and get the most out of their package without compromising on quality of service. Additionally, we send a Star Community Care coordinator to come with you to your 12 months plan review meeting to assist with the size of funding required in the new year.
From our internal structure and processes through to the one-on-one support you receive, we are customer-centred. For example, our care coordinators work with you to understand your needs and the services required to deliver the best outcome for you.
